System level outcomes in Victorian hospitals: is knowing patient’s full hospital history making a difference to clinical decision making?

The project will describe the extent to which Victorian patients access emergency care at multiple hospitals. In addition to quantifying the extent to which patients access multiple hospitals, this will demonstrate the impact of measuring care outcomes from the state perspective as opposed to single site indicators such as: • Readmission rates when a patient represents at a different hospital following treatment; • Death rates following an episode; and • Risk profiles identified through ML/AI when a patient has prior episodes of care or co-morbidities diagnosed at a different hospital.
